If you would like to receive CD's of our services on a regular basis let us know and we will mail you CD's every two weeks. You will be billed at the end of each quarter. The cost of the CD's is $24.00 per quarter ($36.00 outside the U.S.A.) which also includes shipping & handling.

These CD’s are all MP3 compatible 32-kbps which can be played on your computer or MP3 stereos.

They can also easily be converted back to wav files which will play in any CD player.

  Make sure you include all of the information we need to process your order, like name, mailing address, phone number, and email address. Please don't forget to tell us the CD number of the message you want to receive (if the CD number is not available, as in some of the older studies, please order by the date. Both number and date are found at the end of the requested study).



• Snail mail  - Please print and fill out this order form if you will be placing your order via regular mail and send it along with your check.
